Postoperative Care of Patients Undergoing Lung Resection
نویسنده
چکیده
Due to frequent pre-existing comorbidities and variable impairment of postoperative respiratory function thoracic surgical patients still encounter various complications which in high risk individuals may contribute to poor outcome. Postoperative management of patients undergoing lung resection may vary from simple and transient monitoring of vital functions in the recovery room to the highly specialized therapeutic approach that requires direct admission to the ICU. Proper care of these patients focuses on obtaining an early recovery from anesthesia and mechanical ventilation, achieving optimal pain control, preventing postoperative pulmonary complications, and facilitating early mobilization. The early post-operative period following extensive surgical resection is a crucial time when major cardiac and respiratory adverse events may significantly affect both the hemodynamic response and “residual” lung performance. Among the worst complications an early onset of acute hypoxemia, usually identified as post-resectional ALI or ARDS, has always been considered an ominous sign. This severe pulmonary edema, unrelated to cardiac failure and accompanied with endothelial damage and radiographic pulmonary infiltrates, has been shown to be the leading cause of death in patients undergoing lung surgery. In severe postoperative lung dysfunction conventional mechanical ventilation with protective ventilatory settings is needed to improve gas exchange; in less serious disease noninvasive ventilatory strategies may be safe and effective. Other potentially useful treatments for unresponsive hypoxia include the administration of inhaled pulmonary vasodilators, ventilatory support in the prone position, and the application of extracorporeal membrane oxygenation (ECMO). A multidisciplinary management of high risk patients undergoing lung surgery, focused on adequate preoperative “pulmonary rehabilitation”, intraoperative lung protective strategies, postoperative optimization of analgesia and incentivation of cough effectiveness, is essential for a positive outcome. Changes in Blood Gases and Hemodynamic Parameters in Patients Undergoing Lung Resection Surgery and Its Clinical Implications
Introduction: Lung resection surgery is a challenge to thoracic surgeons. Outcome of surgery depends on patients’ tolerance to reduced lung volume and hemodynamic alterations. The present study aimed to investigate the changes in blood gases and hemodynamic parameters in patients undergoing lung resection surgery and the associated clinical implications. Materials and methods: This study includ...
متن کاملAnalysis of the Results of Pulmonary Resection by Minimally Invasive Thoracoscopy for the Surgical Treatment of Lung Cancer
Introduction: Lung cancer is the disease of modern era, and the rate of lung cancer mortality is three times as high as that for prostate cancer and twice as high as the rate for breast cancer. We aimed to analyze the results of pulmonary resection in patients with NSCLC by minimally invasive thoracoscopy. Materials and Methods: We studied 10 patients with NSCLC scheduled for surgical resectio...
متن کاملDetermination of the Effects of Digoxin on the Right Ventricular Function in Patients Undergoing Pneumonectomy
Introduction: Pneumonectomy is the standard treatment of lung cancer, even though patients should undergo several evaluations before surgery; deterioration of cardiopulmonary function after pulmonary resection is inevitable. We have evaluated the effects of digoxin on the improvement of right ventricular function and prevention of probable complications after lung resection surgery. Materials a...
متن کاملEarly respiratory therapy reduces postoperative atelectasis in children undergoing lung resection.
BACKGROUND Early physiotherapy reduces pulmonary complications after lung resection in adult patients. However, the effectiveness and the techniques used in postoperative physiotherapy in children undergoing lung resection have not been well described. Therefore, the standardization of a physiotherapeutic attendance after lung resection in children is necessary. METHODS This was a retrospecti...
متن کاملEffect of Lung Manual Hyperinflation (MHI) on Oxygenation of Patients Following Abdominal Surgery and T-Tube Support
Background: Postoperative pulmonary complications (PPC) are of the major reasons for death. Prolonged mechanical ventilation (PMV) and delayed extubation are leading to the incidence of more seriously complications. The effect of hyperinflation has not been investigated in control of these complications in patients who have been weaned from mechanical ventilation and are undergoing T-tube suppo...
